Project Description

This project represents past work of the UNH Institute on Disability and is no longer active.

The goal of this project was to identify and work to overcome the barriers that prevent Medicaid-eligible older adults and adults with disabilities, particularly cognitive/behavioral issues, from remaining in their own homes and communities. The focus was on "aging-in-place." The project was designed to develop and/or enhance systems that encouraged and fostered collaboration and cooperation of existing housing and support networks. Partnering organizations included, from DHHS: Bureau of Elderly and Adult Services and Bureau of Behavioral Health; NH Housing Finance Authority; Granite State Independent Living; and Dartmouth Psychiatric Research Center. There were five areas of focus: development of partnerships and coordination activities, technical assistance for Elder Wrap Teams, education and training, evaluation and quality assurance, and implementation of a specific demonstration project.
