Project Partners & Funding
Key collaborators include:
- Caregiver Program Manager at NH Department of Health and Human Services, Bureau of Elderly and Adult Services (BEAS)
- Institute on Disability at UNH
- The two pilot ServiceLink sites - ServiceLink Resource Center of Grafton County and ServiceLink Resource Center of Monadnock Region
- The three additional sites joining the project - ServiceLink Resource Centers of Belknap, Rockingham and Merrimack Counties
- Gateways Community Services
BEAS receives and oversees the expenditure of the Title III-E funds from the federal government and is committed to moving the assessment and eligibility determination to the local level. The Institute on Disability has partnered with BEAS and the ServiceLinks in developing the training and evaluation components for the program. Gateways designed the individual budgeting procedures and processes the payment of invoices and payroll. A monthly management team meeting is held, including all partners, to facilitate smooth implementation of the program, develop policies and procedures, problem solve, and improve communication among partners.
