Home Care Connections
Project Description
The goal of HCC is to identify and work to overcome the barriers that prevent Medicaid-eligible older adults and adults with disabilities, particularly cognitive/behavioral issues, from remaining in their own homes and communities. The focus is on "aging-in-place." The project is designed to develop and/or enhance systems that encourage and foster collaboration and cooperation of existing housing and support networks. Partnering organizations include, from DHHS: Bureau of Elderly and Adult Services and Bureau of Behavioral Health; NH Housing Finance Authority; Granite State Independent Living; and Dartmouth Psychiatric Research Center. There are five areas of focus: Development of partnerships and coordination activities, technical assistance for Elder Wrap Teams, education and training, evaluation and quality assurance, and implementation of a specific demonstration project.
